Eligibility for Citizenship Amendment Act?

They will have to show that they have lived in India for five years.
They will have to prove that they have come to India before December 31, 2014
It has to be proved that they came to India from their countries due to religious reason
they have to fulfil other requirements also

Then only they are eligible to apply and after application govt of India will take decision after all verifications whether to give them Indian Citizenship or not.

How to apply for Citizenship Amendment Act?

Application will be submitted completely in online mode for which web portal will be provided
